Home Program
Your Place - at Your Pace
The Home Program is different from our other Cognitive Stimulation Therapy services because it’s a coaching program for family members and support staff. Designed by us and delivered by you, it helps improve engagement, build confidence, and create meaningful moments at home.
Prerequisite Initial Consulation
The Initial Consultation is an assessment conducted at home or via telehealth, where we take the time to understand your loved one — their needs, interests, and routines. Along with assessing whether CST is a good fit, we provide practical, personalised strategies to help them stay engaged and connected at home. That way, you’re supporting your loved one’s cognitive health whether you choose to proceed with our programs or not. The Initial Consultation costs $299 + travel. Program packages are set according to NDIS price guidelines - contact us for more details.
What's included?
12 x personalised session plans to be delivered by family or support staff
1-hour home visit to introduce the session plans and provide training
A telehealth check-in that you can book at any time during the program
A phone check-in after the 12 sessions to discuss next steps (e.g. additional session plans)
Step-by-step resources and guidebooks to make implementation easy and effective
Fully funded through NDIS or HCP (if eligible)
NDIS recommendation letter available to help with funding requests

How to Enrol in the Home Program

After we understand your loved one’s needs, we will invite you to book an Initial Consultation.

We’ll design 12 session plans for you and thenvisit you again to introduce the program and help you start.
